You are a staff nurse in a primary care provider’s office. You utilize computer charting and electronic medical records. Consider the case study below and answer the questions that follow. Mrs. S. is an 83 year old woman with multiple medical problems, including chronic obstructive pulmonary disease (for which she sees a pulmonologist), atrial fibrillation and systolic heart failure (for which she sees a cardiologist), hypertension, diabetes, depression, and mild dementia. She comes to your provider’s office today with a new list of medications. Her pulmonologist changed her inhaler from one brand/strength to another brand/strength, but she is not sure why. Her cardiologist took her off warfarin, and again she is not sure why. The provider asks you to gather all relevant information from her other providers and let him know what changes were made and why. Because the other providers do not share the same electronic medical record your office uses, you will have to gather much of this information manually.
