Implementation Plan

Assignment 2: Implementation Plan
In this module, you will implement your identified intervention described in your research plan. You will physically go out into your chosen clinical setting and implement your interventions identified in your capstone project. You only have one week to implement your plan, and one week to evaluate the plan’s effectiveness, so plan to your time wisely!
Purpose: The implementation process moves the strategic plan from a dust covered book into practical steps that propel changes. You only have one week to implement your care plan, so begin small—say a small group of 5–10 people.
Resources for Implementation: The following resources are necessary to develop a working implementation plan:
• Stakeholder buy-in (nurses, managers, etc.)
• Educational Materials: pamphlets, handouts, posters, a PowerPoint presentation
• Assessment Tools: questionnaires, surveys, pre-/posttests to assess knowledge of participants at baseline and after intervention
• Technology: technology or software needs for education or implementation of a process change
Tasks:submit a 3–4-page implementation plan addressing the following items:
Identify a Change Theory that you would use to implement your proposed solution. (16points)
Describe the process for implementation using SMART goals. (12 points)
Identify at least 4 outcome variables you would use to measure or evaluate success. (12 points)
Explain how the proposal solution or implementation utilized leadership, communication, and collaboration skills to improve a clinical problem in your setting. (8 points)
How long do you think it will take for the results to be seen? How do you plan to implement this intervention? Do you have any specific strategy in mind? What resources will you need? (if applicable)
