Part I: Calculating RN Full Time Equivalents (FTEs)
As the manager, you have been asked to provide an RN staffing plan for a 40-bed in-patient nursing unit with an Average Daily Census of 40. The proposed staffing ratio is 1:6 (1 nurse for each 6 patients per shift). Using the Nursing Hours Per Patient Day (NHPPD) methodology, calculate the number of FTEs needed to staff the unit for a 14-day pay period. Use the information in the Welch and Smith (2020) article to assist with your analysis.

  1. Convert Nurse Patient Ratio to NHPPD.

  1. Determine the nursing unit annual volume in patient days.

40 * 365 equaling to 14600

  1. Determine total NHPPD for the previous year.

The hospital has a staffing ration of 1:6 making it 4 even for the previous years.

  1. Determine number of FTEs needed to staff the unit.

FTEs is calculated by multiplying , the number of rooms by the number of hours per day then multiplied by days per week that will equal to the total hours to be staffed per week. The total in this calculation is then multiplied by the number of people per room that will amount to the total number of working hours per week that is later divided by forty hours to find the total number of fulltime equivalent. In our case, number of room is one with an average daily consensus of 40. Using the average working hours of America, the average working hours is eight hours per day with exception of health care givers who can work up to ten or twelve. From the ration given, the average working days is estimated to be four days in a week. To get the total staffed per week;

12 * 1 that will equal to 12

12 * 4 will equal to 48, total hours staffed per week

48 * 40 equal to 1920, the total number of working hours per week

FTQ will be 48, calculated by dividing the totals above by 40 hours.

  1. Explain why the unit uses a particular staffing ratio and how that ratio is determined.

This unit uses a specific staffing ration in order to offer quality services to patient and ensure that there side-bed relay of shift to ensure that services are patient-centered. This ration is determined by dividing the FTEs staff with FTEs physicians.

  1. Explain whether other units in the same facility would have the same staffing ratio or different staffing ratios. Justify your response.

Other units may have not the same ration to this unit’s staffing ration as the unit’s staffing ration is dictated by the American Nursing Association. 

Part II: Calculating Salary Costs

Based on the FTEs you calculated in Part I, calculate the salary cost for each nurse FTE using the hourly wage multiplied by 2,080 (number of hours worked per year), then add 30% to cover benefits. Use the following to make your calculations: RN salary: $32.50 per hour + 30% for benefits = $42.25 per hour.

  1. Determine the salary per year for one RN FTE.

The salary of one RN FTE is calculated by multiplying the number of hours by the rate of payment per hour. That is 42.25*48 that will total to 2,028 per week, in a year, there are 54 weeks that will total to 109,512 dollars.

  1. Calculate the total salary costs for the entire RN staff to determine your initial professional staff budget for the entire year.

For the entire nursing staff, the total cost will be 109,512*7 that will total to 766,584 dollars.

  1. Explain the importance of budgeting when making staffing decisions.

Budgeting will help the hospital administration to allocate resources and to ensure smooth running of the hospital. Budgeting will also help in determining the number of staff needed and how they are going to be scheduled throughout the year.

Part III: Creating a Staffing Chart for 4 North Day Shift

Using the FTEs you have determined you need in Part 1, create a staffing plan for a 14 day pay period for the 4 North day shift.

(Remember that a nurse who works 72 hours per pay period is a 0.9 FTE; 48hrs/week is a 0.6 FTE; and 24hrs/week is a 0.3 FTE.)
