Identify a Community Health Assessment Model. (This document from the CDC lists commonly used community assessment models:https://www.cdc.gov/stltpublichealth/cha/assessment.html ) What are some benefits of using a specific model to assess a community? What agencies should be included with the assessment? What is a reasonable time frame in which to complete a community wide assessment? What is the role of the public nurse in implementation of the chosen model?

Provide an analysis of your surveyed community and what health-related community need is present. Define the methodology of the health improvement program you will use to solve this problem also include a time line. Include any higher level of care management and policy decision that APNs who work in the community would need to be partner with either community-based consortiums or health care policy makers
Review and reflect on the family assessment tools you have seen this week. Then using a family assessment tool of your choice develop an assessment plan for a family that includes a child or adolescent, young or middle aged adult, and a senior citizen. Explain how this type of assessment will be beneficial in a family centered practice. Be specific and use examples.

Describe a health promotion model used to initiate behavioral changes. How does this model help in teaching behavioral changes? What are some of the barriers that affect a patient’s ability to learn? How does a patient’s readiness to learn, or readiness to change, affect learning outcomes?
Describe one innovative health care delivery model that incorporates an interdisciplinary care delivery team. Explain how this model is advantageous to patient outcomes.
